As you can see, HMS Incomparable has an order to turn west. She's had this order for the last 2 minutes, but has refused to follow the order. Her sister ship, HMS Inflexible, has the same orders, and has followed them without question, though she is relatively undamaged.
I first noticed the problem after she took a second torpedo (the first did relatively little damage). I assumed that it must have damaged her rudder or something...
...But there's no indication of any such damage in the log. Normally, when the rudder gets jammed, it says so next to the maximum speed indicator, but it doesn't say that here. Does that mean that the torpedo damaged the conning tower and it just wasn't reported in the damage log, or is there something else I'm not aware of?
EDIT: I just played on for about 5 in-game minutes, and she's just executed the turn, about 7 minutes after it was initially ordered.

If the rudder wasn't disabled, the most likely cause is misinterpretation of signals.

Yes, but depending on how you're playing the game it can be easy to overlook, and just one misinterpreted signal can send the ship off doing its own thing for quite a bit of time.
