Sounds like she was hit with early-war Mark 13/14/15 torpedoes. I don't think RTW deliberately models such snafus, but American torpedo performance was very nearly that bad early in WWII. Magnetic detonators failed, torpedoes ran deep so contact detonator wasn't triggered, and when the torpedo did make contact, the detonator was crushed before it could set off the warhead. The air dropped Mark 13 also had a tendency to be damaged when it hit the water.

If I recall correctly "Hit by a dud torpedo" events are registered on the torpedo hit counter, but probably either none or neglible damage. But even with some potential duds, that's still supernatural.

While this might be a bug, I think there is a lot of "under the hood" calculations for damage when a torpedo hits a ship. I've had a few similar events in SAI games when a ship took a torpedo and suffered incredibly negligible damage.
