Checking back on the game, these bugs are still in the ship designer:
1. Changing to "sloped deck" armour scheme does not update the weight. Changing to the other two armour schemes updates the weight. 2. The conning tower armour can not be changed in a refit. Thus neither a "Warspite update" nor a proper carrier conversion is possible.
Also the speed curves are still strange. Why not adopt one of the two mods which fixes them?
And maintenance costs and ASW value shown in the ship designer would greatly improve the TP destroyer design process.

3. "Clear mounts" in the torpedo tab does not update the ship graphics.
4. Drawn lines in the ship graphics sometimes are still crooked, especially when zooming in.
edit: The mirrored right side sometimes is lower than the drawn left side, especially noticeable for horizontal lines.

5. When loading a design which is supposed to be built by a foreign yard, "Build at yard in" is not actually updated to reflect that and has to be manually reselected to take effect (eg for the weight calculations).

That said, given that the examples you list show that it was done historically (and given that the whole conning tower is replaced and relocated in a CV conversion), it's valid to question whether the rebuild system should completely forbid CT rearmoring, but that should be posted in suggestions, not bug reports, where it might be overlooked.

I posted it in the suggestions some months ago, but it fits the "bug" definition as well and it is rather immersion breaking to have carriers with 15 inch armored conning towers ("bugs" can originate in the design phase as well, not only during the implementation phase).
6. Some early AI destroyer designs are so overweight, that the player can not build them.
7. Torpedo protection level can be adjusted up (and down beneath 0!) even when no TPS is researched. This is especially tiresome when you check foreign yards for their TPS level. You have to select the country, then up the TPS level, then click the red checkmark to see whether they have that TPS. Then repeat until you find their TPS level. Instead of restricting the selection, you could just display the maximum TPS level for that country to the right of the up and down buttons. Eg Torpedo defence | 0| +- (max 3). Since TPS is so important for most ships beyond DDs, it would be nice to see it in the country drop down menu. Eg Build at yard in USA (24kt, TPS 3).
