Especially late game when torpedo ranges are high I get a lot of "friendly fire" incidents where my own ships are hit by torpedoes from my DDs etc. The problem here is NOT that the AI fires to often. No, the problem is that the "range clear" check (which, if negative, gives you a "friendly ship in ine of fire" when you want to fire torpedoes manually in captains mode and presumably is also done by the AI) is made on the direct bearing from the firing ship to the target, not on the actual firing bearing. If the target ship is moving at speed and the range long those two bearings can have a large difference.
The irony here is that when you want to fire your torpedoes in captains mode you cannot take shots which are perfectly safe since the actual firing bearing is not at that friendly ship "in bearing" and most often it is moving with a course and speed that would take it well clear of the torpedo(es). But when the AI fires at long ranges it can take shots which go directly through your own ships because of the logic error in the "range clear check".
At the very least the "range clear" check needs to be switched from direct line of sight to target to firing bearing (the blue line you see in captains mode vs. the grey line), with "range unclear" when a friendly ship is on that bearing. This should be relatively easy.
Ideally however the relative motions of your own ships would be taken into account (by calculating virtual firing bearings on them) and the "range unclear" would result when any virtual firing bearing to a friendly matches or is close to the one to the intended target.

I had my first friendly fire incident in Rear Admiral mode. Is that working as designed? Some factors: the crew were elite but not special-trained for torpedo warfare, the ships were light cruisers early (1905-ish) and using submerged torpedoes, and strangely enough it was two ships from the same division that fired in line abreast mode and one hit the next.
I can provide the video feed of it if anyone wants (just from an LP on my youtube channel). I currently suspect that it's WAD but just thought I'd pose the question based on tbr's experience in Captain's mode.

If friendly ships maneuver in unexpected ways, it is quite possible to hit a friendly ship, though it should be rare.
