Oh yes saw that one! I suppose the cockpit is a complex environment and even the pilot gets confused sometimes! I semi follow air crash investigations and some of the errors are ridiculous. Like continuous pulling the plane to a stall.. Or left engine fire so shut down the right engine! There's reasons ofc but the end result is like that lol. Although compared to road travel and some of stuff ya see or avoid(wooot..), air travel's still much much safer for sure!

A cockpit is a very complex environment and that is why they have two pilots working together as a team to fly the plane. However, when you test a pilot repeatedly and he fails probably the most important area of his flying, his ability to deal with emergency situations. You now have the responsibility to remove him from flying at least put him in the second seat with a veteran pilot. If he still fails, fire him. Don't wait until he kills everyone.

Of course when you look at it from this POV it is absolutely true. But there're way worse atrocities in the world! The hatred, selfishness, immoralities, greed etc etc are beyond measure. But heck, it is a part that makes humans. But... guess you prefer driving now, oldpop. To each his own I suppose!
