Here is one I've never seen before. I'm not sure if you'll be able to reproduce it from the autosave. [Edit: actually I checked from the autosave and it happens immediately if you advance the time a couple minutes.] I didn't touch any of the destroyers since this surprise attack started, nor did I touch the flotilla attack order. All I did in this scenario was increase the B division to max speed and point it toward the immobile German ships; one click only. Seems like DD Mutsuki went and got in a gun duel with coastal artillery (something of a bug in its own right) and then didn't know what to do while sinking.
